Building Usage for External Ministry Events

The forms below are Acrobat fill-in forms; you may complete them using free Acrobat Reader software and electronically send them to the church, or you may print them, complete them by hand, and then mail them to the church. Questions? Just email the , and we’ll be glad to help.

Step 1. Review the fees associated with use of the facility. If you have questions, please just email the office and we'll be glad to help.

Step 2. Read and understand the policy that is relevant to your event.

Step 3. If you are in agreement with the fee structure and the usage policy for your event, read, sign, and return the Church Usage and Hold Harmless Agreement. Complete, sign, and return the last page of the Event Fees document; this will serve as your contract.

Step 4. Complete the Event Request Form below. When you click Submit, the form will automatically be sent to the church office. Drawing the room setup you would like and then emailing or mailing that graphic to the church office is also a good idea.

By clicking the submit button, you agree that you understand the building use fees and policies.
